2. W4- Employee’s Withholding Allowance Certificate
Note: When completing the W4 Form, select #5 or #7.

Select #5 if You (may meet one or all conditions):

• Are Single and have only one job (enter 0 or 1)
• Are Married/Head of household (refer to your taxes)
• Have Children
• Have another job (verify this form with your other Employer)
• May earn $4000 by the end year (enter 0 or 1 unless you have children)

Select #7 if You (must meet both conditions):

• Didn't owe Federal taxes last year AND won't owe Federal taxes by the end of the year
• Don't file Federal taxes (i.e. Public Assistance, SSI, Disability etc)

3. If you meet (ONE) of the following conditions below you must complete the IT-2104-E- Certificate of Exemption from Withholding:

• 18 years old
• Full time Students less than 25 years old
• Did not owe NYS taxes in the previous year/ won't owe NYS taxes this year
• Parents claim you on their taxes
• Parents don't file taxes (i.e. Public Assistance, SSI, Disability etc)

If not, you must complete the IT- 2104- Employee’s Withholding Allowance Certificate

Required Identification (see sample lists below)

Please Submit 1 item from List A with your JJC Validated ID Card

OR

Submit Your JJC Validated ID Card with an additional item from List B AND 1 item from List C (see below)

List A

1. US Passport
2. Permanent Resident Card or Alien Registration Receipt Card (Form I-551)

List B

1. Driver’s License, State ID or Learners Permit Card (from US) w/ photo & demographic information.
2. John Jay Collage School ID with Validation sticker
3. Voter’s Registration Card
4. U.S. Military or Draft Card
5. School record, Report Card; Clinic or Hospital Card (If you are under 18 years of age)

List C

1. Social Security Card or Certificate of Birth Abroad (Form FS-545)
2. Certificate of Report of Birth (Form DS-1350)
3. Employment Authorization document issued by Department of Homeland Security
