Required documents for the undergraduate application process include:

1. UMOS I.D. and official NYPD transcript

2. $70 Application Fee (payable by check, money order, or online)

3. Official transcripts for all post-secondary transcripts, including military

4. Immunization records against Measles, Mumps, and Rubella

The application process can be conducted on-line or in-person (by appointment only). The on-line application process is a two-step process (verification and application process).

OPTION 1: Application Process (On-Line)

Part I: Verification Process

A.NYPD Leadership Form:
Complete and submit the following NYPD Leadership Form by clicking here.

B.NYPD Verification:
Request an official NYPD transcript (signed and sealed) to be sent to the Undergraduate Admissions Office at 524 W 59th Street, Room L. 64, New York, NY 10019.

C. College Transcripts:
Submit your official college transcript(s) (signed and sealed) to the Undergraduate Admissions Office.

Part II: Application Process

A.Online Application:
Once we receive all of the documents [online NYPD Leadership application, NYPD transcript, and college transcript(s)], we will send you an email directing you to complete the online application with appropriate instruction.

B.Application Fee**:
Submit a non-refundable, $70 application fee. Application fees can be paid online using a debit or credit card or submit a check or money order to the Admissions Office, payable to CUNY/UAPC.

**Please note the application fee is waived for military personnel. In order to be waived, you must provide proof of Veteran/Military status (i.e. DD-214, Current Military ID Card, or Deployment or Pre-separation orders)** 

C. Immunization Records:
Once the application process is submitted and complete and the payment is received, you are required to submit proof that you have been immunized for measles, mumps and rubella as well as complete the meningitis section of the immunization form. You have the option of faxing the immunization records to the Student Health Center at 212-237-8026. If you opt to send the fax, please follow-up with the office to confirm receipt at 212-237-8053.
